Information for a patient

The procedure of preparation for keratoplasty surgery

In advance the patient informs us about the problem – by phone or by mail. We give answers to your questions and offer to send by e-mail already available medical records (ophthalmologists’ checkups, examination results). This information is studied by surgeons-ophthalmologists of the Dnipropetrovsk Regional Clinical Ophthalmological Hospital "DOKOL" and the patient is informed about the preliminary conclusion about the possibility of keratoplasty, and are specified the convenient dates of his or her visit to the hospital.

If you do not have at hand the results of examinations and diagnostics – in the Dnipropetrovsk Regional Clinical Ophthalmological Hospital "DOKOL" you will be held a full-scale diagnosis and examination of your eyes with the help of modern diagnostic equipment, and also laboratory tests will be made, if necessary.

Diagnostics consists of:

- Visual acuity check of the patient, including in the absence of accommodation;

- Carrying out on Scheimpflug camera: identifying the binocular vision, autorefractometry and keratometry; pachymetry (measurement of the corneal thickness) and corneal topography (identifying individual parameters of the corneal surface);

- Echobiometry (ultrasound examination of the eyeball on a biometric device Carl Zeiss IOL Master);

- Gonioscopy and fundus-lens examination.

Based on the results of diagnostics passes the conversation with the doctor, where you will get the answers to all your questions about your problem. Taking into account the individual characteristics of your disease, your doctor will explain all the stages of implantation and will appoint the operation. The method of anesthesia is discussed with the patient. The choice of anesthesia method depends largely on the patient's age, state of health and pain sensitivity. Keratoplasty, carried out in the hospital "DOKOL", is performed under general or local (retrobulbar) anesthesia with the latest safe medicines.

Waiting time for the operation is usually from 2 to 5 days. In some cases, a patient's cornea needs preparation for keratoplasty. Therefore, the waiting time for the corneal graft is spent on preoperative treatment.

You are admitted to the hospital for keratoplasty.

   The patient is admitted to the hospital 1 day prior to surgery.

   The patient is examined, these examination results are studied. 

   The surgeon-ophthalmologist plans the procedure, the parameters and techniques of conducting the operation.

In the operating room

   At the beginning, a team of anesthesiologists work with the patient – to provide adequate anesthesia required for a successful operation.

  Using a femtosecond laser or microsurgical instrument is generated the corneal graft.

   In the second step, using the femtosecond laser or microsurgical instrument is produced a through layered keratotomy of the cornea of the patient. All manipulations are carried out under the supervision of a microscope with a jeweler's precision.

   In the defect, after the removal of the cornea, is gently imposed the corneal graft, after which the patient's own cornea edge with help of very thin surgical threads is sewn with the extreme part of the corneal graft.

   After the surgery, the surgeon checks the hermeticity and uniformity of the joints, then, follows the imposition of bandages on the eye, or a special protective contact lens.

  After the surgery, the patient remains under the supervision of ophthalmologists and anesthetists at least for 12 hours. Then, the patient requires daily observation by the ophthalmologists of the hospital at least within 7 - 10 days. After this period, the doctor appoints the dates of the subsequent examinations and the patient returns home, which contributes the most comfortable recovery. After keratoplasty patients are given recommendations on physical regime and prevention of eyes from negative influences (high temperature, chemicals, and others). The recovery period is long (from three months to a year). Sutures are removed usually 6 - 12 months after the plastic operation.

Postoperative recovery

   As a rule, the outcome of keratoplasty is favorable: cornea is left transparent, the desired visual functions appear. In exceptional cases, the implant failure is possible, which is shown through intense redness of the eye and corneal opacity, but the applied medication helps to avoid such displays. A second operation may be conducted in case of need.

   When the initial state of the visual analyzer makes it impossible to achieve the desired optical performance after the plastics, and in order to get rid of possible postoperative astigmatism, the patient may need to hold the laser correction.

   During the recovery, the patient is under regular supervision of his or her doctor.
